About Dr. Vasisht: Credentials, Training and Philosophy

Bhupesh Vasisht, MD, FACS, is a board-certified plastic surgeon and a Fellow of the American College of Surgeons. He grew up where he practices: southern New Jersey. He earned his undergraduate degree from Rutgers University and his medical degree from UMDNJ–Robert Wood Johnson Medical School. His surgical training spans Boston Medical Center, Staten Island University Hospital and a plastic and reconstructive surgery residency at UMDNJ–New Jersey Medical School, where he was appointed Administrative Chief Resident during his final year.

After residency, Dr. Vasisht completed a highly specialized hand and microsurgery fellowship at the University of Connecticut–Hartford Hospital, then returned to Boston University School of Medicine as a faculty member. He served as Assistant Professor of Surgery and Chief of the Division of Microsurgery, teaching the next generation of surgeons while continuing to operate at the highest level.

He has published peer-reviewed research, contributed to surgical textbooks and lectured internationally. He is credentialed at Virtua Health System and Jefferson Health System, and performs procedures at Centennial Surgery Center, Summit Surgery Center and Millennium Surgery Center.

Which Type of Implant Is Right for You?

This is one of the most common questions patients ask during the breast augmentation consultation process. The best choice depends on your body, your goals and your lifestyle. Here’s what Dr. V will walk you through during your consultation:

Saline Implants Filled with sterile saltwater, saline implants can be placed through smaller incisions and are FDA-approved for patients 18 and older. If a rupture occurs, the saline is safely absorbed by the body. They tend to feel firmer than silicone and may be a good fit for patients who want simpler long-term aftercare.

Silicone Gel Implants Most patients who want the most natural look and feel choose silicone breast implants. FDA-approved for patients 22 and older, modern silicone implants are designed to maintain their integrity even in the event of a rupture. The feel is closer to natural breast tissue, and they tend to move more naturally with the body.

Gummy Bear Implants A subtype of silicone, gummy bear implants use a highly cohesive gel that holds a teardrop shape even if the shell ruptures. They’re a popular choice for patients who want a very natural-looking contour with long-term viability.

Fat Transfer (Fat Grafting) If you’re looking for a more modest, completely natural enhancement without implants, fat grafting may be the best option for you. In this approach, fat is harvested via liposuction from the abdomen, flanks or thighs, purified and then carefully injected into the breasts. Results are typically subtler than implants, but for the right candidate, the outcomes look and feel extremely natural.

The Importance of Implant Shape, Profile and Placement

Deciding on the best type of implant for you is just one of several decisions that go into producing the results you’re looking for.

Shape refers to whether the implant is round (offering fuller upper pole projection) or teardrop-shaped (mimicking the natural slope of the breast, with more fullness at the bottom).

Profile describes how far the implant projects from the chest wall. A low profile offers subtle enhancement while a high profile creates more dramatic projection. Women with narrower frames often benefit from a higher profile to get the volume they’re looking for without going too wide.

Placement is one of the most clinically important aspects of the procedure. Submuscular placement (beneath the pectoral muscle) provides more natural coverage and tends to look better over time, particularly for patients with minimal natural breast tissue. Subglandular placement (over the muscle, under the breast tissue) may be appropriate for patients with sufficient existing tissue and can offer a slightly easier recovery.

What Type of Incision Does a Breast Augmentation Require?

Surgeons have multiple locations to choose from, each offering unique advantages at the clinical and patient-preference levels.

The location of the incision affects where any scarring will be, how easily the implant can be placed and what your revision options look like down the road.

The inframammary incision, made in the crease beneath the breast, is the most widely used approach. It offers excellent visibility and precise placement, and the resulting scar sits in a natural fold where it’s rarely visible.

The periareolar incision runs along the lower edge of the areola. When it heals well, it can be nearly invisible, though it may not be suitable for all implant types or sizes.

The transaxillary incision is placed in the armpit, leaving no scar on the breast itself. It requires a different technical approach to implant positioning and is typically recommended on a case-by-case basis.

Breast Augmentation vs. Breast Lift: Which Do You Actually Need?

This is one of the most important questions Dr. V addresses during consultations, and the honest answer is that implants and a lift do different things.

Implants add volume. A lift repositions tissue and elevates the breast to a more youthful position on the chest wall. If your breasts have sagged due to aging, pregnancy or weight fluctuations, implants alone may not be enough to produce the results you want. In many cases, combining augmentation with a breast lift gives you the fullness you’re looking for, along with improved shape and position.

What to Expect from Your Breast Augmentation Experience

The Day of Surgery

Breast augmentation is performed as an outpatient procedure at our accredited Centennial Surgery Center or a trusted hospital facility. The surgery typically takes one to two hours under general anesthesia or sedation. You’ll go home the same day.

The First Few Days

Expect some tightness, soreness and swelling. Dr. V uses the On-Q pain pump system, which delivers numbing medication directly to the surgical site for the first three days, dramatically reducing the need for oral pain medication and making the early recovery far more comfortable. Most patients are surprised by how manageable this period actually is.

Resuming Your Routine

Many patients return to desk work within three to five days. Strenuous exercise and heavy lifting should be avoided for four to six weeks to allow the implants to settle properly.

Results Timeline

You’ll notice an immediate increase in volume, though swelling will obscure the final contours for several weeks. Implants begin settling into their final position around six to eight weeks post-surgery, and full results, including natural softness and movement, are typically apparent by three to six months.

Am I a Good Candidate for Breast Augmentation?

The best way to know for certain is to come in for a consultation. That said, ideal candidates are in good general health, have realistic expectations about outcomes, are non-smokers (or willing to stop smoking before and after surgery), and are not currently pregnant or breastfeeding. The FDA approves saline implants for patients 18 and older and silicone implants for patients 22 and older.
